מה ניתן ללמוד ממחקר ארוך טווח בסרטן הכליה

כאונקולוגים, אנחנו עדים לשינויים דרמטיים המתרחשים בעשור האחרון בעולמות הטיפול בסרטן כליה גרורתי. על מה, אם כן, מומלץ שנבסס את ההחלטה הטיפולית עבור מטופל ספציפי?

ארממנטיום הטיפולים הזמינים בסרטן כליה גרורתי השתנה בעשור האחרון בזכות טיפולים יעילים חדשים רבים שנכנסו לארסנל הטיפולי. האסטרטגיה הטיפולית הנוכחית המומלצת מבוססת על משלב של שתי תרופות אימונותרפיות או אימונותרפיה ומעכב אנגיוגינזה או תרופה אנטי-אנגיוגנית כטיפול בודד. עם זאת, אין הגדרה ברורה של הבחירה בסוג המשלב הטיפולי.

מטרת הטיפול, גם במצב גרורתי, היא ריפוי, אך אם ריפוי אינו בר השגה, אזי הישרדות ארוכת טווח היא מטרה סבירה, בהקשר של גורמים פעילים רבים.

מה ניתן ללמוד מנתוני מחקר Checkmate 214 Phase 3 Trial?

מעקב ארוך טווח של הניסוי המרכזי, CheckMate-214, הראה כי משלב אימונותרפי עם Opdivoי(Nivolumab) בתוספת Yervoyי(Ipilimumab) שמר על יתרון הישרדותי על פני Sutentי(Sunitinib), בחולים עם קרצינומה מתקדמת של תאי כליה (RCC), בזמן מעקב מינימלי של חמש שנים.

ממצאים עדכניים שהוצגו לאחרונה בשני כנסים חשובים: IKCS ו-ESMO 2021, הראו שכמחצית מהמטופלים שקיבלו את המשלב נותרו בחיים לאחר 5 שנים.

החוקרים הציגו תוצאות מעודכנות של יעילות, בטיחות ותוצאות הישרדות מותנית מ-CheckMate 214 עם משך מעקב מינימלי של 5 שנים (זמן מעקב חציוני של 67.7 חודשים), המייצגים את המעקב הארוך ביותר בפאזה שלישית, שדווח עבור משלב אימונותרפי ב-aRCC.

במחקר checkmate 214 חולקו באקראי 1,096 חולים עם סרטן כליה מתקדם/גרורתי לשתי זרועות. זרוע של המשלב האימונותרפי Opdivo 3mg/kg+Yervoy 1mg/kg הניתן למשך 4 טיפולים מידי 3 שבועות ולאחריו טיפול ב-Opdivo 3mg/kg פעם בשבועיים* וזרוע של מעכב אנטי-אנגיוגני Sutent הניתן במינון 50mg פעם ביום למשך 4 שבועות ולאחריהם שבועיים הפסקה בכל מחזור של 6 שבועות. המטופלים חולקו לפי דרגות סיכון לפי IMDC criteria כאשר ה-primary endpoint של המחקר הינה עבור מטופלים בדרגת סיכון בינוני/גבוה.

לאחר חמש שנות מעקב 34 מתוך 547 חולים (6%) בטיפול בזרוע Opdivo+Yervoy ו-9 מתוך 535 חולים (2%) שטופלו בזרוע Sutent המשיכו בטיפול. טיפול מערכתי שלאחר מכן (sequential treatment) התקבל על ידי 55% מהמטופלים בזרוע Opdivo+Yervoy ו-68% מהמטופלים בזרוע Sutent.

אנליזת שרידות OS של חולים עם מחלה בסיכון בינוני או גבוה הראו תועלת מתמשכת בטיפול ב-Opdivo+Yervoy לעומת Sutent (חציון של 47 חודשים לעומת 26.6 חודשים; HR = 0.68; 95% CI, 0.58-0.81). שיעורי OS לחמש שנים היו 43% בטיפול במשלב Opdivo+Yervoy ו-31% בטיפול עם Sutent.

הסתברויות PFS לחמש שנים של חולים עם מחלה בסיכון בינוני או גבוה שטופלו במשלב Opdivo+Yervoy לעומת Sutent היו 31% לעומת 11%.
גם בניתוחים של אוכלוסיית Intent to treat נשמר יתרון ב-mOS לטובת Opdivo+Yervoy (55.7 חודשים לעומת 38.4 חודשים; HR = 0.72;י95% CI,י0.62-0.85), OSי5 שנים (48% לעומת 37%), ORR (39% לעומת 32%), שיעור תגובה מלאה (12% לעומת 3%) ומשך התגובה (חציון לא הושג לעומת 24.8 חודשים).

כמו כן, משלב Opdivo+Yervoy הציג פרופיל בטיחות שניתן לניהול ולא נמצאו אותות בטיחות חדשים בתקופת מעקב הארוכה יותר.

מהי הבשורה החשובה שנוכל לבשר לחולים?

ממצאי המחקר מראים, כי גם לאחר חמש שנים מתחילת המחקר עדיין לא הושג משך התגובה החציוני בשילוב של Opdivo+Yevoy. ממצא זה מעיד על ההשפעה המתמשכת של המשלב האימונותרפי הכפול וזאת בשורה חשובה עבור חולים החיים עם מחלה זו.

בפרסום זה נותחו לראשונה תוצאות של ניתוחי הישרדות מותנית (conditional survival), המוגדרות כהסתברות מטופל להישאר בחיים (cOS), ללא התקדמות (cPFS), או בתגובה (cDOR) למשך שנתיים נוספות מעבר לנקודות הזמן השנתיות. מדד זה משמש לניבוי תועלת מתמשכת לטיפול ומספק מידע פרוגנוסטי כמותי.

הנתונים חדשים שהוצגו, מראים ששיעורי cOS cPFS ו-cDOR גבוהים יותר בעקביות עם Opdivo+Yervoy לעומת Sutent בכל קבוצות החולים.

בקרב חולים שנותרו בחיים 3 שנים לאחר תחילת הטיפול, ההסתברות להישאר בחיים למשך שנתיים נוספות הייתה גבוהה יותר בקבוצת Opdivo+Yervoy לעומת Sutent (81% לעומת 72%).

בקרב חולים שנותרו ללא התקדמות במשך 3 שנים לאחר התחלת הטיפול, ההסתברות להישאר ללא התקדמות למשך שנתיים נוספות הייתה גבוהה יותר בקבוצת Opdivo+Yervoy לעומת Sutent (89% לעומת 57%).

בקרב חולים שהגיבו לטיפול ונשארו בתגובה במשך 3 שנים, ההסתברות להישאר בתגובה למשך שנתיים נוספות הייתה גבוהה יותר בקבוצת Opdivo+Yervoy לעומת Sutent (89% לעומת 66%).

תוצאות אלו מדגישות את היתרונות הקליניים המתמשכים שנצפו לאחר 5 שנות מעקב אחרי טיפול במשלב Opdivo+Yervoy, לעומת טיפול ב-Sutent, בחולים עם aRCC. כמו כן, התוצאות מעידות כי רוב החולים שנותרו בחיים, או שהתגובה אצלם נמשכה לאחר 3 שנים, ימשיכו לחיות או יישארו בתגובה גם לאחר 5 שנים עם משלב Opdivo+Yervoy.

אם כך, מהם השיקולים שעליהם עלינו לבסס את ההחלטה הטיפולית?

לאור נתוני מחקר Checkmate 214 Phase 3 Trial שהוצגו לאחרונה, יתכן שנוסף לנו פרמטר חדש: אמינות. לטעמי, מחקר שמציג יציבות בתוצאות לאורך 67 חודשים הינו אופציה טיפולית שניתן לבחור בה בבטחה. הנתונים החדשים שהתקבלו במחקר מאפשרים להתייחס למספר נקודות שיסייעו לנו בקבלת החלטה מושכלת של סוג המשלב הטיפולי, בהן:

  1. למשלב Opdivo+Yervoy נתוני מעקב פאזה שלישית הארוכים והבשלים ביותר בין כל סוגי המשלבים בטיפולי RCC.
  2. תוצאות המחקר של המשלב Opdivo+Yervoy מעידות על תועלת יציבה ב-OS/PFS במשך כל שנות המעקב (זמן מעקב חציוני של 67.7 חודשים), כאשר ה-HR עבור השרידות יציב סביב 0.7 לאורך שנים.
  3. ישנה תגובה מתמשכת (Durable Responses) במשלב Opdivo+Yervoy המאפשרת לחולה להמשיך בחייו כאילו מדובר במחלה כרונית.
  4. במחקר נצפתה הפוגה מלאה בקרב 11-13% מהחולים שטופלו במשלב Opdivo+Yervoy, והנתון מעורר תקווה שאולי מדובר בריפוי.
  5. בטיפול במשלב Opdivo+Yervoy קיים פוטנציאל להפסקת הטיפול ללא פגיעה בתועלת.
  6. נצפה במעקב ארוך טווח כי לטיפול במשלב Opdivo+Yervoy יש סבילות טובה ורעילות כרונית נמוכה.

אם כן, במצב בו המטרה שלנו היא להשיג Durable Response עם איכות חיים טובה של החולה, הפחתה של רעילות כרונית וכמובן שיפור בשרידות - משלב Opdivo+Yervoy בהחלט יהיה בחירה טובה.

Opdivo approved indications:
Unresectable or Metastatic Melanoma:
Opdivo as monotherapy or in combination with ipilimumab is indicated for the treatment of advanced (unresectable or metastatic) melanoma in adults.

Adjuvant Treatment of Melanoma:
Opdivo is indicated for the adjuvant treatment of patients with melanoma with involvement of lymph nodes or metastatic disease who have undergone complete resection

Metastatic Non-Small Cell Lung Cancer:
- Opdivo, in combination with ipilimumab and 2 cycles of platinum-doublet chemotherapy, is indicated for the first-line treatment of adult patients with metastatic or recurrent non-small cell lung cancer (NSCLC), with no EGFR or ALK genomic tumor aberrations.

- Opdivo is indicated for the treatment of patients with metastatic non small cell lung cancer (NSCLC) with progression on or after platinum-based chemotherapy.
Renal Cell Carcinoma:
-Opdivo as a single agent is indicated for the treatment of patients with advanced renal cell carcinoma (RCC) in patients who have received prior anti-angiogenic therapy.

-Opdivo in combination with ipilimumab is indicated for the first line treatment of patients with intermediate or poor risk, advanced renal cell carcinoma (RCC).
-OPDIVO, in combination with cabozantinib, is indicated for the first-line treatment of patients with advanced RCC
Classical Hodgkin Lymphoma:
Opdivo is indicated for the treatment of adult patients with classical Hodgkin lymphoma (cHL) that has relapsed or progressed after:

* Autologous hematopoietic stem cell transplantation (HSCT) and brentuximab vedotin, or
* 3 or more lines of systemic therapy that includes autologous HSCT.
Squamous Cell Carcinoma of the Head and Neck:
Opdivo is indicated for the treatment of patients with recurrent or metastatic squamous cell carcinoma of the head and neck (SCCHN) with disease progression on or after platinum-based therapy.

Microsatellite Instability-High (MSI-H) or Mismatch Repair Deficient(dMMR) Metastatic Colorectal Cancer:
Opdivo, as a single agent or in combination with ipilimumab, is indicated for the treatment of adult and pediatric patients 12 years and older with microsatellite instability-high (MSI-H) or mismatch repair deficient (dMMR) metastatic colorectal cancer (CRC) that has progressed following treatment with a fluoropyrimidine, oxaliplatin, and irinotecan.

Urothelial carcinoma:
-OPDIVO is indicated for the adjuvant treatment of patients with urothelial carcinoma (UC) who are at high risk of recurrence after undergoing radical resection of UC.

-Opdivo (nivolumab) is indicated for the treatment of patients with locally advanced or metastatic urothelial carcinoma who:
- have disease progression during or following platinum-containing chemotherapy
- have disease progression within 12 months of neoadjuvant or adjuvant treatment with platinum-containing chemotherapy.
Hepatocellular Carcinoma:
Opdivo, as a single agent or in combination with ipilimumab, is indicated for the treatment of patients with hepatocellular carcinoma (HCC) Child-Pugh A who have been previously treated with sorafenib.

Small Cell Lung Cancer (SCLC):
Opdivo is indicated for the treatment of patients with metastatic small cell lung cancer (SCLC) with progression after platinum based chemotherapy and at least one other line of therapy

Esophageal carcinoma:
-OPDIVO is indicated for the adjuvant treatment of completely resected esophageal or gastroesophageal junction cancer with residual pathologic disease in patients who have received neoadjuvant chemoradiotherapy (CRT).

-OPDIVO is indicated for the treatment of patients with unresectable, advanced, recurrent or metastatic esophageal squamous cell carcinoma after prior fluoropyrimidine- and platinum-based chemotherapy

Malignant Pleural Mesothelioma

OPDIVO, in combination with ipilimumab, is indicated for the first-line treatment of adult patients with unresectable malignant pleural mesothelioma.

Gastric Cancer, Gastroesophageal Junction Cancer, and Esophageal Adenocarcinoma

OPDIVO, in combination with fluoropyrimidine- and platinum-containing chemotherapy, is indicated for the treatment of patients with unresectable advanced or metastatic gastric cancer, gastroesophageal junction cancer, and esophageal adenocarcinoma.

Yervoy approved indications

Unresectable or Metastatic Melanoma:

  • YERVOY (ipilimumab) is indicated for the treatment of advanced (unresectable or metastatic) melanoma.
  • YERVOY (ipilimumab), in combination with nivolumab, is indicated for the treatment of patients with advanced (unresectable or metastatic) melanoma.
